To assess the clinical and operational implications of integrating chiropractic services in healthcare settings, we conducted a multisite qualitative case study of nine U.S. private sector medical facilities that introduced chiropractic services in their organizational context. The case study design included semi-structured interviews and review of archival records. Key stakeholders included clinic managers, clinical staff, and patients. Data collection was conducted between December 2015 and February 2016.
The impetus for introducing chiropractic services varied among sites. In most cases, the addition of chiropractic was a strategic decision made by facility leadership or an interdisciplinary committee. It was generally implemented in a well-coordinated process, involving multiple stakeholders over an extended period of time. Most sites adapted existing human resources, physical plant, and information technology infrastructure to provide chiropractic services, with the majority having expanded their chiropractic workforce in response to patient demand. Several sites implemented continuous quality improvement processes designed to support chiropractic integration and monitor patient outcomes and satisfaction.
Clinics were established within a variety of existing clinical service areas, with most sites allowing direct patient access through provider referral or self-scheduling systems. Despite high patient demand, most sites experienced efficient clinic operations. However, a number of sites encountered challenges in establishing their chiropractic programs, including cultural resistance to chiropractic integration and clerical/administration systems that were unfamiliar with chiropractic coding parameters and insurance reimbursement processes.
